1.) Click on the blue poisonus mushroom on the overworld. However, for the submap you start at, I'd recommend changing the standard graphics because ExGFX just gets loaded properly when entering the submap/main map via pipe or star road.
2.) Click on the Mario head and you'll see the dropdown menu "SNES Registers and Level Modes". Mode 03, 04, 07, 08, 0A and 0D are for vertical levels. You can see the appropriate informations -like actual layer 2 use for vertical levels- under the dropdown menu.

Click the Change Properties in Header button (Mario head) and change the option in the topmost dropdown box. Level type 0A is a normal vertical level, and 08 is a vertical level with layer 2 level data. You can see if you have chosen a vertical level type by checking if below the dropdown box it says "Layout: Vertical".

1. Click on the blue poision mushroom in the OW. The rest works just like inserting level ExGFX.
2. Click on the Mario head and set the value in the first menu to 0A.
