Err.. stacked munchers, cutoff, a-easy-to-get-1-up-mushroom, and probably predicting bad level design.

Already mentioned; cutoff, stacked Munchers.. In the new screenshots: overuse of Note Blocks and an absolutely terrible lava palette.
You have a bit of work to do before this hack will meet our standards, but don't just give up because of a little criticism. Too many potentially great hackers have left the community because of that. In a variation on a quote from Chuck Jones, you have a thousand terrible SMW levels in you, and the sooner you get them out the better.

Clearly I made this post too early!
Thanks for all the suggestions. I'm reading the Hack Removal Log right now to see what other hacks have been rejected for. Most likely I'll have to gut the whole thing and start over, but I don't mind. I made about eleventy zillion changes just to get that far, and probably would've ended up gutting whole levels anyway.
(Although personally I don't think the teal lava looks that bad.)

Teal lava is okay, if it's not also mixed with reds, oranges, purples... that stuff. Mixing a palette like that tends to make the lava look like a weird acid trip, and it stands out in a bad way.
