Kinda similar to the other thread I had made a while back, but this time I decided Kaizo shouldn't count, and also I think enough time has passed.
I haven't made that many non-Kaizo levels, but this version of Punishing Pothole might be it. Also pretty difficult is this level that will be included very late in my own hack Combo Breaker, which looks very similar to the title screen.

Pretty difficult to finish without savestates. I will probably dumb it down just a bit.
Also, I don't want this to turn into 'Vanilla Design Contest discussion 2.0'

Well...I think it is pretty obvious what the hardest non-kaizo level I have made is. In fact, when I made a countdown of the 5 hardest non-kaizo levels, I put my level on that list. You can see my countdown post here: Click Me!
If I ever use my level in a hack, it will be the final special stage (AKA the Super Ultra Secret Level)

Well I'm pretty sure it's this level from the Special World of my hack. (And probably just about every other level other that).
Level Video
I might have toned it down just a bit but not really much at all.. (It's possible savestateless)

Is there a download link? I would like to try that level out!

Watch this
One of my first levels, made when I was new with good level design (I know the design is bad). It was a hard level but it looked messed up on the status bar and stuff.

I have no clue why, back then, I thought this was good difficult level design. Wow. (Ignore the short, lazy intro in the video, too)
Hell, that level probably qualifies as Kaizo.
(I never included it in a finished project, though)

Probably "Serenity Castle" from a super ancient hack that was also my first. I can't really remember much from the level, but what I do remember is that it was >10 minutes in length, hard-to-pass enemies all over the place and some really difficult jumps and underwater sections and all those things. Savestate spamming was used. A lot.
Now that I mention it, it might just be me who sucked at playing. But at the very least, it's the hardest level I've made in proportion to my platforming skills.

It can actually qualify as non-kaizo, as long as you enable a side exit incase the red switch has not been hit (unless the player is unable to enter the level from the overworld without the red switch)
Also, that multi-powerup block should just be replaced with a cape so the player cannot screw up.

Most of my levels aren't terribly difficult. Although I have had complaints that the second room of my latest vanilla level contest entry is too difficult.
If I had to choose a level of mine that is most difficult I'd probably pick a world six level from Lost in the Forest is probably one of the hardest levels I've made (Although Evilguy said that it was a little easy for world 6). Also for my upcoming hack Boris world 3 level 4 has a vertical autoscroll part that's pretty tricky and the the first special world level of Boris has a swimming part that I think is pretty tough to beat.

My hardest level abused precision jumps which was making the level a bit more mean than difficult. I was a newbie with those things so I didn't bother it was fair or not.

By precision jumps, do you mean jumping from 1 tile wide platforms? I don't see how that is hard...only repetitive.

Jumping in between munchers? That's hard for normal players.

Oh that is what you meant. I thought you meant jumping from tile to tile.

No... By "precision jumps" I meant hard platform jumps or jumping over an obstacle with a lot of precision. Mostly spinjumping on enemies to get past some big obstacles.

My hardest level would probably have to be Mario Castle, from the first hack I ever really was slightly devoted to.
At first, it was fairly easy, but what followed was precision jumps involving jumping across the heads of Koopas, falling platforms, and to top it all off, switching to spin-jumps from normal jumps and vice versa. Nothing would stay in the same spot for more than a second.
I really hope I can out do it in the final castle of the hack I'm making so far, namely because the hack I'm working on involves a good deal of skill from world 3 onward. At least, that's the plan.

Aether's Rotary Room, while not difficult in the sense that it requires quick reflexes and requires precise movement to survive, has stumped so many people in Let's Plays of the game. That one puzzle is the sole reason I included a puzzle solution file with the hack.

I'm going to say the level "What a hard Fortress" in my old hack was very difficult but not kaizo.
