What used to scare me in video games:
-Phanto in SMB2
-Temple Tempest in DKC1, or any level featuring chase scenes
-Most boss fights
What still scares me in video games:
-playing a 3D Mario game and I'm in a really high up location, trying to be careful not to fall
-the music in DKC3's Lost World (Krematoa map)
-The second half of Eversion (not gonna spoil it for anyone who hasn't played it yet)
-the eternal staircase in SM64 (it's actually fun because it's scary)
-the last level in the room escape game 'diversity' if you're going for the bad ending.
It's a bathroom filled with blood and ominous music. The scariest part of that is, if you set fire to the rat and use the burning rat to short circuit the washer, its ghost will haunt you if you look inside the washer, taking up most of the screen while moving around, and making a loud noise.
What used to scare me in video games was some sprites in SMW, the munchers and sprites in SMB3. Now what does scare me still is some bosses of the Mickey Mouse game (like the one in the snow world), and yeah, you know, some sprites can still scare me if they surprise me.
As a kid, the mad piano in SM64 scared me a fair bit. I'm sure there was more but I don't really remember stuff like that, and I was rather young.

From what I can remember:

- Phanto, SMB2
- Guard cough, Goldeneye 007
- Crocodile Cacaphony music, DKC2
- Shadow Temple, The Well and some other locations in OoT. Some places in Majora's Mask were also pretty scary.
- The spider chase (Mimi?) in Super Paper Mario.
- Bowser's barking sound when you enter the castle for the first time in SM64.

Not really scared of things in games anymore, although I really dislike seeing disgusting things like enemies drenched in a pool of blood and that type of stuff.

I hate that too. I'm not a fan of things like this. I think it just looks horrible. Not sure what did scare me if I was a kid.

Anyway, I believe "Ghost of Rattman" from Portal 2 is sufficiently scary enough. It's a surreal music track with Rattman's high-pitched psychotic rambling entering halfway through.
When I was a little guy, I was scared of SMB1 nes castle music...

Umm... *cough*... Iggy in SMW.
As a child, I used to be absolutely terrified of the Chain-Chomp in Super Mario 64, and pretty much everything in the Boo Mansion stage.

It's funny, there hasn't been another game that has terrified me as much as it did. Probably because I was only around six or seven when I first played it.
Stuff that still scare me in video games... That sketchy girl in FEAR 1 and 2!! She wasn't that bad in FEAR 3 though. The only time I ever played it too was when I was alone and when it was like midnight or 1... Yeah, not the best idea haha.

And when I was a little kid, I was scared of the Legend of Zelda Dungeon music... yeah for real.

I have always also been scared of things popping out at me around corners... Dead Island kinda scared me a couple of times, due to damn zombies jumping out of corners and grabbing me. Bastards...

Well, here's some funny little fears of mine:

-Snacker the Shark in Banjo-Kazooie (Until I realized he doesn't do anything scary in particular)

-Lava in general in 3D games

-The thought of facing 3D Bowser in Super Mario 64

-Zombies at night, after watching someone play Resident Evil 3 in the local Pokemon Card League. Not sure why I watched it in the middle of a Pokemon Card game between turns, though.

I'm not certain why I was so afraid of the first three things, but I believe I was around 9 or 10 for all of them. Now I'm hardly scared by ANYTHING in even horror games.
Oh, I almoost forgot:
the sound that happens when you get to that door after the first level of chrono trigger 1.
(It used to scare me a lot back then, now it still does but not as bad.)
-The Moon from Zelda
-G...i....y...g...a....s...... from Earthbound
-The Piano from SM64. Really freaked me out.
-Dunno why, but Ganon and Bowser used to scare me.
-Gold - Pokemon Lost Silver. Great, couldn't sleep for 2 days 'cause of you.
