1-Plan your level

You should think what type of level you want. Remember this have to be fine with your overworld.

:) 2 - Make your level

Use the buttons in the bar. If the grass button's pushed, you can edit the objects. The green shell button edits the sprites. To open the Add Objects Window, push the Turn Block Button. If you want to add sprites, push the Blue Baby Yoshi button.If you want to edit the background, push the Question Block Button (To add tiles) and the Leaf button. If you don't want (example) a grassland level, but a Ghost house, you can push the Blue Poisonous Mushroom Block and select the tiles. If you want to edit the music and time, put a layer 2 (Spikes of Wendy's Castle,etc.) and the vertical scrolling, push the Mario's Head button. If you made a water level, or put a fireball, or the sprites interact with water and lava, you have to push the Lakitu button and enable sprite buoyancy. For layer 3, you push the Water button. To edit layer 2, press the Yellow Ground button.

  You can put ExGFX, too.

You can see the correct palette too. Play your rom in Znesw e make a savestate. In YY-chr, press F-12 keyboard button(after you selected the option 4bpp SNES). Now you can edit your animated tiles and you can see the correct palette. Pm me if you don't understand.

When you're using blocktool, make sure if the blocks are inserted sucefully (without message errors). Then Open Lm and search the tile you've added the block. Create a block with the 8x8 tiles and make it act like tile 25, 130 or 132. Then paste the block in the space that has shown a purple description. Example: If the message tells "Smashable Brick), you'll creat a block and make it act as block 132 (if you want to turn it into a coin if the Blue-Pow is actived) or block 130, never block 25!

If this doesn't work, I can't help you anymore, because I'm not sure if the BTSD works.

There are a few steps for adding custom blocks to your hack:

1 - Download the Blocktool or the BTSD (I don't know work with this);
2 - Save a level in LM of your ROM;
3 - Open Blocktool and the Windows Calculator;
4 - In Blocktool, open your Rom (after step 2) and click in Add Block button; now, choose your blocks;
5 - In Calculator, push the Hex button and type the Map16Number you want to the blocks (Lm works with Hex, but the BT works with Dec) and then pull the Dec button. Copy this number and paste in the Add Block dialog;
6 - When you finish the steps 4 and 5,save the blocks. Go to file and click on save;
7 - Make sure there wasn't errors when you was adding blocks;
8 - If these steps are sucessfully, open your ROM in LM and click in the Map16 Tile editor and the Add Objects window.
9 - Go to the pages where you added your blocks and search purple descriptions. These descriptions describe the custom blocks.
10- Now, in the Number of custom blocks in page, make a block (with GFX or ExGFX) and select its acts like (ONLY acts from pages 0 or 1, or there will create an ASM loop!) and paste it in the Custom Block Space.

Now you can use your block!!!
If it doesn't work, PM me. I'll create a patch to you with your desired blocks.

Open a GFX file in YY-chr with the Waterfall animation. You can find it in Super Mario Saga 1 The Last Adventure.
Put the ExGFX in the folder, open LM and insert the EXGFX.
Click on Red Poison Mushroom button, go to Extend animated area GFX dialog and search the animated waterfall ExGFX.
Click on the SMB3 brick button and edit the animation (4 8x8 : 16x16, normal; Destination E0 and frames, you'll select the upper left tile on the page of ExAnimation (Page 5; tip on your keyboard after entering on 8x8 editor: ctrl+alt+pagedown).

If it doesn't work, PM me. I'll send you the ExGFX, a palette file and a txt file with the correct frames to add the waterfall.

How do I edit ASM to make the giant ? blocks to spawn a mushroom, for example??? And how do I insert ASM to make the giant bricks (these two blocks seen in SMB3 world 4)?

There isn't a Map16Page if you've drawn your GFX.

You'll need to make the tiles and press F9 and then F2. F2 will create a Map16Page file.
