Looking at this error is seems like you added some data to your cfg that you should have not: each custom sprite should have exactly 2 extra property bytes. That sprite uses "extra bytes", which are different from the extra property bytes: you add them in the "extension" field of Lunar Magic's sprite insertion dialog, not in the cfg file.
As far as I know "#amk 1.1" isn't a valid header, you should either use 1 or 2. Besides that, petitemm has an error with ties: instead of writing "c1^2", for example, it will write "c1^c2", which is what's making channel #0 sound out of sync. So you should clean it up by removing the note names after all the tie symbols.
That happens because you didn't remove the test levelasm files that come with UberASMTool, just remove "105 test.asm" and "106 test2.asm" from your list.txt (and also remove those files from the level folder since they're useless).
