Tip: After editing your overworld, make sure to use an EMPTY save file, as save files with progress may load Mario's position based on previous versions of the overworld.

The fireballs are a nice mechanic to an interesting sprite. What I noticed, though, is that when the bullet bill is set to explode on stomp, it doesn't check whether it should explode or spawn fireballs (so it's inconsistent in a certain way). You also can't whack the bullets with the cape regardless whether they can be reflected or not. The bullet bill also falls down when spawning fireballs even though it makes better to use a different death animation (e.g. a smoke).
Wow this is really amazing. I especially like the exploding and the fire spreading one. That's what was really missing on this site, good job ! 5 stars.
