When you touch this block, it will warp you to a different OW/submap. This is a better version of my old block, I didn't know how to work with 16bit address then. I also explain how to get Mario to a specific point in the OW in the asm file.
I haven't tried this block, but I'd suggest you to use Lunar Magic's secondary entrance overworld features, it should be easier than mess up with a custom block. With that you can specify a fixed location in the overworld and just warp to it from the level (using a door or a teleport block).
Thank you for the suggestion. After a bit of work I figured out the secondary exit warping.

I must have messed something up on this one. I inserted it using GPS and it registers correctly in my 16x16 editor but it does not warp me when I hit the box. Any suggestions?
