After taking a look in Lunar Magic, the problem turns out to be that the event that triggers when completing Gemtoa Cliffs 3 is the same event that triggers when you complete Island Ruins, and the secret exit is shared with the regular exit of Hillside Passage.
To fix this, you have to open the hack in Lunar Magic and manually change the event number of Gemtoa Cliffs 3 from the faulty C into the intended 4F. Then it works perfectly fine.

I have come across a massive problem with the 1.50x version of Super Mario Omega, on Gemtoa Cliffs 3 of world 6 when I did both exits it wouldn’t let me progress at all no matter how I completed the level which is quite disappointing because I was actually starting to really enjoy it up until that one part.
