This is a very close attempt, but there are some issues I need to point out before accepting.
The biggest reason: The chord sample is tuned wrong. With the range of the note's pitches you entered specifically for this song, it's supposed to be $02 $68 to suit the chords.
And while you're using chord samples, you're also supposed to use a major chord sample too along with the minor chord you used from samples of insanity/individual samples/synth/misc folder, because from what I've heard, the song uses both major and minor chords. Now, where to put a major or minor sample in #5:
For a+, put the minor chord. For g+ or f+, put the major chord.
Another option: If you want a simpler way without taking the time placing instrument calls on the notes, replace the chord sample with a sample that plays an interval of fifths. The one I can think of off the top of my head is sample 07 from SNES game Equinox, which is string instruments playing 5th interval. It's pretty quiet though, so make sure to raise the volume up a lot when using that sample. The tuning also is $02 $68 for that sample.
Aside from that, minor suggestion: the volume of the slow guitar slide on the high range may want to be toned down a bit, it sticks out.
Also, to not let the open hi-hat sound too short and cut off-y, set the tuning to $00 $00. I've been seeing you adding some kind of tuning value instead of zero on the noise instruments for a while now, so I'm not entirely sure if making it sound that short is your intention or not.
Though, like I said, this is a very good attempt otherwise! Fix the harmonized sample and I think it's acceptable.
