I'm working on a hack right now, and I used the sample level that was included for some of the tilesets as the base for a handful of levels, but I made sure to make a full level on top of the sample level part.
Would my hack be rejected for plagiarizing someone else's level design?

Sample levels are usually available for everyone to use and base their tilesets off of, so no, they don't count as plagiarizing in this case (or simply put, using others' work without permission). You're fine in that regard.
