I have a level that starts from level 105, then it goes to level 106 via a teleport block. Is it possible to restart the level from level 106 when the player gets a midway point bar? I tried but Mario dies immediately when I enter the level....
No, the midway entrance is always in the level first entered from the overworld. You can, however, place the midway entrance in a small area closed off to the rest of 105, containing a pipe leading to 106.

In this case, if a midway point is put at level 106, Mario restarts the whole level from the midway entrance of level 105, if he had hit the midway point previously, obviously, and as far as I know, there isn't a way to make him restart the level from level 106... sorry. :/
