Absolutely eye-searing animated palettes, floating and stacked munchers, danger upon level entry (steep slope into oblivion), unfair invisible coin block placement, stuck until you commit suicide if you fail at getting past the first muncher wall at the first try (unless there are no powerups in those blocks, in which case, forced death unless you enter with one), unfair enemy placement (Koopa falling from above the screen borders).

Glitched graphics, bad editing of the Switch Palace message, Mario overlaping the message, maybe use of coin question blocks in a Switch Palace, and also maybe, blatant level edit.

Raised-up switch make Mario overlap the message, miscellaneous issues with the message itself (wrong sprite graphics for hardcoded blocks, blocks covering the text, minor case of bad grammar), glitched bonus game due to coin block usage in switch palace, bad placement of coin blocks (how are you supposed to hit that upper left one?) extremely blatant Yellow Switch Palace edit.

EDIT: Beaten...

EDIT 2: I knew I had seen that picture before. I guess it being from a removal log explains it.
