It's the poison mushroom sprite.
Read mikeyk's readme.txt, it explains it.

Oh lol, I already got the poison mushroom in the game. I thought that was like, some kind of block that went with it. My bad. Unless it is?

No, it's the poison mushroom sprite. It is just a *.bin files instead of *.asm, that's all.

Oh! Well if you want to do that, easy. Use
these blocks
by
Milk
.
Just use the one for a
normal
(not custom!)
sprite and put in 85. Bam, there you go.
