I think what is inside of question blocks is, there is an interdimensional vortex that goes to one of a few planets. Those planets contain either starmen, mushrooms, flowers, capes, Yoshis, coins, etc. When the block is bumped, that activates the vortex and an item from one of those planets is transported to the outside of the block. Right after that, the portal closes, except for some blocks that give out coins, where the portal stays open a little longer.
