Is there a "donut lift" block? If not would it be possible as a block and not a sprite?

Can be found in the sprite tool folder. There also one in ASM Format:

However, you still have to use a sprite for the activated donut lift (it's and included with sprite tool).

The block still requires the custom sprite to be in the level, right?

The block spawns the sprite automatically; you don't need to place it yourself.
