This was a nice and honest attempt for a vanilla type hack. I'd say the main problem with your hack is that it doesn't offer anything of interest while also being rather short. This equals to about 5-7 minutes of playtime and the gameplay and level design, while not really flawed aren't interesting either.
You have 3 levels with actual content to them (that is to say not just a message box and side exit). 1 of them is a switch palace that is in all honesty rather horrible. I guess it was suppose to be a maze or something of that sort. If it was: Nobody likes mazes (in SMW), if it wasn't: What was it even there for? Switch Palaces don't have to be amazing levels but they still shouldn't be boring like that.
As for the other 2 levels, they were ok, but nothing special. At least noting I'd reject this hack for. It's just in combination with the overall shortness.
There is also a weird glitch in which you can instantly die when riding yoshi and going on the left wall bouncy thing below the turn block bridge even if you don't touch the bridge. You get sucked into the side of the pipe and die instantly sometimes. Kinda hard to reproduce.
As a general improvment, I'd say make your levels a bit longer. Otherwise you did a good job, it's not all the linear and you added bonus rooms and all. Speaking of bonus rooms, the door in the first level kinda makes you skip almost the entire level...
Events creating cutoff is not really a problem (if there remains no cutoff by the end of the event) but it is still something that should be avoided if possible.
For example, you could make 2 islands rise rather than one.
Another problem is the black space appearing in the background in your vertical level. Set your BG to scroll slowly vertically (v-scroll) in

(By the way, the 3 exits are not counting the switch palace and the two side-exit-enabled levels.)
Don't know what the first two are about, no problems on my end.
The third is also not a problem, it's more what your event does rather than taking too long.
Also, you DO count secret exits for the total exit count. Switch palaces are a point of debate but are usually counted as well.
