This hack is... pretty weird overall. I think you have the wrong idea of what a creepypasta hack should be. Also, I think that this distracted you from the core factors, there are lots of causes which made me reject this hack.
First, there is not enough gameplay per single run. Your hack's levels are extremly linear, samey, and not too different each other. Plus, you went with 6 exits, which isn't enough if you consider how your levels are. I suggest you to try making each level more unique from the others (adding gimmicks and things like this, instead of being a "go right while avoiding enemies" all the time), as well as, maybe, making your future demo more lenghty (something like 10 exits, if you will keep the same kind of design, should be enough).
Second, there is not good enough quality content. There isn't a clean use of palettes and graphics overall. A big offender may be this, but I could make an example for each level. Again, going in the creepypasta way of hacks doesn't justify you from making messes with palettes or graphics.
Other misc errors I found here:
- In the first level, the first enemy you meet is confusing: it looks like you can jump on it, but it's a spiny.
- As I said again, you should take care of the graphics and palettes. Stuff like this:

and this:

doesn't look too good.
- Each time Mario stomps/kicks an enemy, the item in the item box will disappear for an instant. Try to find a solution to this.
Once all the main issues are fixed, I'll accept your hack.
