I think the most important thing to do first is the OW with all of the events completely functional and every level tile correctly configured. Then you can pick a random level in the OW and know the theme, the difficulty, and sometimes even a particular gimmick you will use there.

I plan out everything before I start on the hack.

I make a notepad file, list out all of the patches, blocks, sprites, graphics, music, and ASM for every level and world.

I also make a credits list for each ASM so I don't forget to credit the files I've downloaded.

But I guess the first thing I do in Lunar Magic is the Intro cutscene or the Overworld.

Try and have an idea of what you are trying to make. Then make it.
1) Cry after seeing let's plays of JUMP and VIP Mario 5
2) Make the most creative levels I possibly can
3) Make an overworld
4) Realize it's crap
5) Remake overworld

You have to add humor to absolutely everything.


What I do:
1)Start making levels
2)Blatantly edit the overworld and keep in mind I'll have to change it later
3)Start doing title screen stuff
4)Start crying about how it's too hard
5)Start making a whole bunch of trashy GFX
6)Continue level making
7)Go back to the overworld
8)Edit messages and try to make the OW look good
9)Give up


I playtest during each of these steps, so you should, too.

I actually have two ways of making a hack.

Way 1
_____

(1) Make a new OW
(2) Think my hack sucks
(3) Delete ROM entirely.

Way 2
_____

(1) Start making levels out of pure random
(2) Change the GFX
(3) Keep making levels
(4) Make a new OW
(5) Repeat step 3

Way 2 works better for me. It's also how I got the idea of latest hack.
wtf fam I've never done the OW first
I mean, yes, I plan all the worlds (including themes) I want in my hack, but don't do the OW until I have the levels done, probably because it's easier to get inspiration for the OW layouts with the levels already built, and then I build the OW around the levels.
I design my levels at pure random, then see where they would fit in my hack, and modify them accordingly. Designing the levels in order just doesn't work for me.
Making the OW first always got me in much more trouble, because then you will feel forced to make more levels than you intended (or less).
My method of making hacks is just flat out going balls to the wall—I have absolutely no method to my madness. What happens first is always different.
Levels first, then overworld afterwards.

Usually, I just do whatever I feel like doing, but nowadays, I seem to start with overworlds, probably because I really enjoy making them.
Title screen stuff usually.
I usually go for the levels first to not deal with the hassle of not wanting to create more than needed and actively fit with the map I design. I can worry about the overworld later if I have a general idea of how I want my hack to turn out. Misc stuff like writing texts for message boxes and editing the title screen also come at rather random times.

Exactly what I do.

lolyoshi also makes a good point.

DO NOT DESIGN YOUR OVERWORLD FIRST. If you really like designing them then go harass the staff to hold an overworld contest.

Also, screw exgfx ain't nobody got time for that
1)Ignore editing intro and OW.
2)Ctrl + Del level
3)Create new level layout
4)Add enemies, coins, blocks, power-ups
5)Repeat 3 & 4 for every level
6)Playtest
7)Fix levels that don't work

The only issue i have is that 90% of the time, I don't have the motivation to sit down and work on my hack. Mainly due to no ideas for the level layout. (I like to work on each level in order, to make sure i have edited them all) I edit all of dinosaur lan,d tehnd onut plains, then vanilla dome, then the cheese bridge area, then forest of illusion, then chocolate island, then bowser dome, the star world, then special world.

That allows me to make sure very level has been edited.
