The "free-flight" idea I originally put forth in my Pokemonslaught hack (which was subsequently scrapped due to lack of original level content, as well as being far too easy) could probably be fleshed out by making the level boundaries rather tight (so you can't just fly over everything), and peppering you with lots of flying enemies (such as Paratroopas, Super Koopas and Bullet Bills).
To compensate for this, you'd have two "outs": a limited number of HP (similar to a SMB2-style hit system), and always have a projectile weapon. Other than that, there would be NO powerups (apart from your HP being restored at midway points/start of the next level). Thus, no hiding behind that Starman to give you invincibility through tough sections.
Oh, and the timer would count down at roughly the same rate as in SMB1 (roughly 1 unit ticked off every half-second, rather than 100 units every 60 seconds). You'd have to hurry to get through each level, but also dodge those incoming Bullet Bills!
